A written request must be completed and submitted to the Program Director, La Maison du Parc, by mail, e-mail or fax. This request must be submitted by a social worker, a physician, a health care specialist, or a support group for persons afflicted with HIV/AIDS. In an exceptional situation, a request may be sent to the Program Director by a person in need or by his or her family or friends. The director will evaluate the request and act accordingly.
